Choosing hospice is about prioritizing comfort and quality of life. We guide you through each step, including:
- Hospice eligibility: usually based on a physician’s certification and Medicare guidelines (such as a prognosis of six months or less and a decision to focus on comfort).
- Starting hospice care: our team coordinates directly with your doctor and family to begin services quickly and seamlessly.
The Four Levels of Hospice Care
Hospice care adapts to each patient’s needs. At Aspen Grove Hospice, we provide all four Medicare-defined levels of care:
Ongoing support where you live, focused on comfort and daily needs.
Short-term, round the clock nursing when symptoms require extra support.
For symptom management that cannot be provided at home.
Up to 5 days of short-term care to provide family caregivers with rest and relief.

Our interdisciplinary team surrounds each patient and family with coordinated support, including:- Medical Director oversight

- Partners who provide Durable Medical Equipment (DME), supplies, and medications delivered right to the home
